Public Notice - Symphony Health Trust; Termination and Transfer of Membership

Background

 

Symphony Health Trust, having no Board of Trustees, and thus being incapable and/or unable to carry out its functions and objectives, was placed under the Curatorship of Maemo Mesotlo (in his capacity as Partner of the firm Maemo S D Mesotlo & Associates (“MSD Mesotlo & Associates”), by Order of The Court (UAHGB-000297-17) on 21 December 2017.

 

On 20 February 2018, The Curator, acting on the resolution of Beneficiaries (Members) of Symphony Health Trust passed at the Special General Meeting held on 19 February 2018, invited proposals from suitably qualified and experienced medical aid schemes or other similar entities registered and operating in Botswana, to submit proposals for the acquisition and administration of Symphony Health Trust.

 

Pula Medical Aid Fund (“PULA”) submitted a proposal to the members of Symphony Health Trust which is expected to result in the Trust members transferring to PULA. PULA’s proposal was approved by Symphony Health Trust members at the 2nd Special General Meeting held on 22 March 2018.

 

Termination and transfer of membership

 

Active and paid up members of Symphony Health Trust

 

Members who wish to transfer to PULA will be subject to the terms and conditions of the offer submitted by PULA and enjoy the full benefits of becoming a PULA member which will include:

 

  • Discounted medical aid subscriptions;
  • Reimbursement of members’ refunds in respect of claims paid up front to medical health care service providers;
  • Management, by PULA, of outstanding medical/health care service provider claims to the extent to be confirmed by PULA upon conclusion of the transfer arrangement; and
  • Provision of medical aid scheme administration service.

 

Terminated (past) members of the Trust

 

Terminated members of the Trust will remain personally liable for any outstanding medical health care service provider claims, which accumulated during their membership tenure with the Trust. However, terminated members are welcome to join PULA through the transfer arrangement and their medical health care service provider claims will be managed by PULA on their behalf, the extent of which is to be determined upon conclusion of the transfer arrangement.

 

Terminated members who are already members of PULA are advised to re-join PULA through the transfer arrangement and will enjoy the same benefits as members of Symphony Health Trust who will be transferring to PULA.

 

Medical health care service provider claims

 

Outstanding medical health care service providers’ claims of members of the Trust joining PULA through the transfer arrangement will be managed by PULA to an extent to be determined by PULA and the Trust Curator, upon conclusion of the transfer arrangement.

 

Symphony Health Trust Termination and Transfer Forms and PULA Membership Application Forms will be available for completion from 23 April 2018 to 06 May 2018, at the following address:
